School transition is a process that prepares students, families, schools, and communities to develop the necessary skills, knowledge and relationships to assist students in successfully moving from one setting to another. Transition should not be viewed from only the perspective of the students and families, but rather should entail the perspectives of families, students, teachers, administrators and community members.
