Student Status (annual APH census only)
Students who are registered in your district on the first Monday of the current census year are counted in your district. Change the status of the student if these criteria are not met.
Moved In-State: This category includes students who moved to another district, are home-schooled, or attend private school. Make notes in Section 4 as to where the student moved. Please be as specific as possible. Include contact information for home or private school students.
Moved Out of State
Delete: Not eligible to be enrolled in public school. Please make notes to explain the reason for deletion in Section 4.
Not Legally Blind: Not legally blind or does not function at the definition of blindness.

Basis of Print Disability (choose one)
- Meets the definition of blindness (MDB)
- Functions at the definition of blindness (FDB)
- when visual function meets the definition of blindness as determined by an eye care specialist. Students in this category manifest unique visual characteristics often found in conditions referred to as neurological, cortical, or cerebral visual impairment (e.g. brain injury or dysfunction).
Note: Measurements of visual acuities include:
Distance Vision: a central visual acuity of 20/200 or less (using a Snellen chart or an acuity determined in Snellen equivalents) in the better eye with the best correction
Visual Field: Restricted field of 20 degrees or less
Counts Fingers: Should be used only when an eye specialist finds it is not possible to obtain an acuity using the Snellen Chart
Functions at the Definition of Blindness: Should be used when visual functioning is reduced by a brain injury or dysfunction and visual acuity is not possible to determine using the Snellen Chart

Current eye-reports are considered the most recent date for an eye-report on file at the district, except in the case of initial VI eligibility determination which must be within one year.
Title of Professional Certifying Print Disability
This is usually an ophthalmologist or optometrist for blind students or other medical doctors such as a neurologist for the diagnosis of Functions at the definition of blind. Current Written Plan Type
All public-school students are covered under the state’s written curriculum plan. However, only students with certain individual education plans qualify as print-impaired. certification. The type of certification dictates the funding streams and sources from which materials can be obtained.
Individual Education Program, 504, and Individual Service Plan dates must be updated annually in the registration
Auditory Reader: Students primarily using a reader or auditory materials as their instructional media.
Braille Reader: Students primarily using braille as their instructional media.
Symbolic/Non-reader: Students not working on or toward a traditional readiness level; students not participating in traditional print or Braille reading curricula (symbolic readers); students who do not fall into any of the above categories.
Pre-reader: Students working on or toward a readiness level; infants, preschoolers, or older students with reading potential.
Visual Reader: Students primarily using print as their instructional media.

This documentation can be on any type of education program, reading inventory, or other research-based assessment to determine eligibility.
- Consent on file at the GIMC
The GIMC must have a signed and notarized consent to register students at the American Printing House for the Blind and to acquire accessible educational materials from other organizations. Consents must be signed by the legal guardian or in the case of a student 18 years or older by the student themselves. Consents are only required once and are valid until revoked in writing or guardianship changes. Consents can be submitted to the GIMC through the secure portal on the Educators’ page of the GIMC website or accessed by this Consent Submission Link.
